Can I just get a consult and not ongoing care?
YES! JH knows that sometimes we just need a check in and to get advice on what to do next. We therefore provide a variety of services that can fit this need:
- Annual PT Wellness Exam: JH clinicians know your annual at your PCP is pretty hands-off (we experience the same thing). We therefore provide this option for those who seek an annual check in on the state of their body. During an Annual PT Wellness Exam, your PT will check your strength, range of motion, sensation, balance, and functional abilities relative to your current work, life, and recreation/sport needs and goals, and make appropriate recommendations to keep you active and healthy. Yearly tracking of these measures also provides you trend data that may help with early detection of any changes before they become a greater concern.
-Fitness Program Assessment and Plan: This option is great for those looking to increase their overall fitness, are seeking advice regarding the interaction between physical fitness and a medical condition, and/or maintain strength, range of motion, and function as they age. During a Fitness Program Assessment and Plan, your clinician will review your medical history and your goals, assess your range, strength, and function, and provide you a personalized comprehensive exercise routine.
-Sports Physicals: For adults entering into or participating in recreational sports, JH provides sport physicals to ensure your body is ready to take on the challenge of the game, reducing your risk for sport-related injuries. During a Sport Physical, your clinician will assess your range, strength, power, and agility in all motions specific to your sport, and provide you an exercise routine to improve on *IMPORTANT NOTE: Per CHSAA Bylaws, Physical Therapists are not certifying professionals for youth sports in Middle or High School athletics, and therefore JH is unable to serve our students in that regard at this time. JH is advocating for change in these bylaws.*

About Documentation and Care Collaboration
JH provides all clients full access to evaluation and treatment notes. This is your body, and you own this information. No questions asked.
JH uses HIPAA-secured systems for collaboration between clients, providers, and care partners. JH clients can elect to have their documentation securely shared with any provider or care partner they name.
As JH does not accept insurance, your health insurance company will have no access to any diagnosis or treatment information from JH. We feel this is a high benefit to you, as diagnostic information can follow you in future insurance care claims and needs. Only you can provide this information to your insurance. Please note that if you choose to receive a Superbill from JH and submit an out-of-network provider claim to your insurance for reimbursement, your health insurance company will then have this information.
